Problem

Liz expressed dissatisfaction with her initial care, highlighting the lack of transfer to a specialist stroke rehabilitation ward for immediate rehabilitation. Additionally, she needed a detailed and thorough discharge plan as there was no consideration for home modifications or rehabilitation support, especially regarding stairs in their house. Liz's mobility poses a risk of falling outdoors. Since her injury, she has been unable to work, resulting in exacerbating her social isolation. As a result, her mental health and sleep quality have significantly declined, leading to feelings of sadness and depression.

Furthermore, Liz and Deb experienced distress due to inconsistent care from nurses, who acted in a range of caring to indifferent or rude ways, including moving Liz to another ward without warning or consent and disregarding her need for assistance. On the other hand, Deb also expressed exhaustion with the stressful situation, as she is the sole earner and has had to return to work, making balancing work demands with caregiving responsibilities difficult. Therefore, Deb has decided to seek assistance and support in caring for Liz. Moreover, a health professional's failure to acknowledge Deb's role as Liz's spouse during medical rounds added to Deb's frustration, as she wanted to be included in the discussion about Liz's condition and improvement.
