Prepare a 10-13-question survey to collect primary data regarding your selected problem statement.

Your survey questions should include a mixture of categorical (nominal and ordinal) and quantitative (interval and ratio) variables. You may only use two open-ended questions in your survey.

On the questionnaire, please list the level of measurement (nominal, ordinal, interval or ratio) next to each survey question. Be sure to number each question. You may use only two (2) open-ended questions on your questionnaire. You must write response categories for your close-ended questions.
