Question

Akira, a new patient to the obstetric practice where you work, is waiting in the examination room for the certified nurse-midwife (CNM). Akira is 28, a G2P1, and at 7 weeks 3 days gestation dating from her last menstrual period (LMP). Akira met her active duty husband while he was stationed in Japan 5 years ago, and her mother and brother came to the United States along with them after his deployment a few years ago. Akira and her husband have a 3-year-old son together who is still unaware that his mom is pregnant. As you wait for the CNM, you reflect on your conversation with Akira. She's told you that she has a great relationship with her mother and can't wait to share her joy at the delivery with her as she did with her son when she delivered in Japan. However, unlike Akira herself, who speaks English very well, her mother doesn't speak any English and is very traditional. She is expecting the same experiences and traditions to be followed in this delivery as they were with Akira's first, and Akira is having a hard time explaining the differences. Akira is worried that her mother will be upset.
