Chief Complaint: Palpitations, SVT History of Present Illness: is a 56 y.o. female who is being admitted for evaluation of Palpitations. Pt has PMH of H TN, Depression, HI-DI and by report CAD and Ml (but no record in chart). Pt reports for the past few months (not sure of exact time) she has experienced palpitations, described as a fullness in her chest. Pt reports intermittent chest pain, substernali left sided with no radiation 5/10. Pt reports this palpitations have occurred before and just seemed to go away. She has been to ED twice in the past) received adenosine and HR would improve. Pt reports that there was never a clear cause as to why she Pt denies any SOB, fevers, chills, abdominal pain, but does report nausea and sweats. Pt denies cigarette use, ETOH use, or illicit drug use.
