He started having trouble when walking from the parking lot into the local grocery store about two days ago. His medication list currently includes: Carvedilol 12.5 mg, oral, twice daily Nifedipine 60 mg, oral, daily Furosemide 40 mg, oral, on Monday, Wednesday, and Friday Detemir (U-100) 4 units, daily in the AM Past Medical History Chronic renal insufficiency Hypertension Congestive heart failure Diabetes mellitus type 2, poor control Deep vein thrombosis Alcohol abuse Chronic obstructive pulmonary disease with respiratory failure Physical Examination Height 69 in., weight 205 lb. Blood pressure: 138/88 Pulse 86 beats/min, regular Lungs clear, neck supple negative for jugular venous distention Lower extremities 1 edema Social History Tobacco: 52 pack-years Alcohol: only occasionally now. 10 years ago-hard alcohol 3x/week.
