Patient: 67-year-old Hispanic retired male HPI/ROS: is evaluated for urinary retention, and difficulty starting and stopping his urinary stream. Symptoms have been present for 2 to 3 years gradually increasing symptoms for the past six months PMH/PSH: Hypertension, hyperlipidemia, coronary artery disease. Social History/Substances: Current medications: Atorvastatin 40 mg P.O. Q HS, Bystolic 5 mg. P.O. BID, Lisinopril 10 mg. P.O. QD, aspirin 81 mg P.O. daily. Allergies: Sulfa drugs (rash) Vital signs: T: 98.0F; P: 66 ; R: 16; BP: 128/78 Exam/lab findings: General: awake and alert oriented X3, in no acute distress. HEENT: PERRLA, fundoscopic exam benign. Neck: no bruits bilaterally. Heart: regular rate and rhythm, S1, S2, no S3, S4., no murmur. Lungs: Clear to auscultation bilaterally. Abdomen: bowel sounds present 4 quadrants, soft, non-tender, no organomegaly. Digital rectal exam of the prostate reveals smooth enlarged prostate without nodules, nontender. Recent labs: PSA, CBC, CMP, Testosterone level all within normal limits.
