interventions should improve patient well-being and include any patient/family teaching and nursing care for the patient and family. Interventions should take into consideration the patient's personal health goals and interprofessional health care team collaboration necessary.
Patient Teaching should be described throughout the interventions. Should also include the patient response to teaching if applicable.
Evaluate the plan of care with a focus on patient goals. If timelines prove to not be realistic for your Care Plan, outline how you would evaluate whether your goals and interventions were met and any changes to goals or timelines.
