A 87 years old female who was admitted to a skilled nursing facility with an( ORIF) done on the right hip fracture one week ago. The client has been cleared to go to Rehab for extensive PT and OT therapy. The patient has a stage 3 pressure ulcer to her sacrum that was cultured and found to be positive with (MRSA) Methicillin Resistant Staphylococcus Aureus. The patient can be a little forgetful, heard of hearing, ests adequately at all meals and wears depends for occasional dribblings of urine. She also has non Blancheable areas to her bil. buttock her heels are dark red/ Purple color.Isolation process was implemented but no signs were placed outside the room. The facility did not have any hand sanitizer anywhere except on the medcarts locked up in the bedroom. The nurse also noticed that the doctor went into the room without Downing or glowing. The nurse used the Bp cuff on the patient and gave the CNA to use on the other patients without sanitizing it. The CNA only repositioned patient 2 times in 8 hours shift. Patient developed s/s, of deterioration with a temp of 101.3, with another open area to her heel black color size of a quarter. Another patient down the hall from Jane has developed MRSA. The nurse manager called urgent meeting to help identify quality improvement opportunities for infection control the department.
