Assesment - Practical Assessment
The Practical Assessment is a set of tasks that must be completed in a workplace, or in an environment with conditions similar to that of a real workplace.
To be assessed for this unit of competency, you must demonstrate your skills and knowledge to establish, maintain and evaluate an organisation's work health and safety (WHS) policies, procedures and programs in a work area to ensure a safe workplace, according to WHS legislative requirements. It takes a systems approach and addresses compliance with relevant legislative requirements.
The Practical Assessments in this workbook include:
1. Workplace Project Assessment
A series of written practical tests assessing the learner's practical knowledge and skills underpinning of the unit of competency. This includes the learner completing workplace documents or similar as evidence of competent performance.
2. Workplace Practical Observation
A set of assessment tasks where the learner must demonstrate practical skills relevant to the unit of competency. These skills are to be demonstrated while being observed by the assessor.
Workplace Project Assessment
Project Overview
This workplace project assessment requires you to develop at least one work health and safety management system in your specific work area and implement it on at least one occasion to comply with requirements of your organisation/workplace and WHS laws applicable to your state/territory.
This assessment is divided into thirteen tasks:
• Task 1: WHS Requirements
• Task 2: WHS Management Plan
• Task 3: Approve Resource Required to Implement WHSMP
• Task 4: Hazard Identification Form and Risk Register
• Task 5: Implement Risk Control Measures
• Task 6: Presentation of Resolution of Risk Management
• Task 7: Include Hazard Identification in Organisational Change
• Task 8: WHS Induction Pack
• Task 9: Deliver WHS Induction to Required Personnel
• Task 10: Training Needs Analysis
• Task 11: Provide WHS Training to Required Personnel
• Task 12: WHS Evaluation Report
• Task 13: Modify WHS Management Plan
This project requires you to complete the assessment tasks in a real workplace, or in an environment with conditions similar to that of a workplace.
Each task comes with a set of instructions. You are to follow and perform these instructions while being observed by the assessor and/or submit any required documentation.
Before starting this assessment, your assessor will discuss with you these tasks including instructions and guidance for satisfactorily completing them.
You are required to:
• Complete the tasks within the time allowed, as scheduled in-class roll.
• Adapt, adopt and communicate WHS policies that define the organisation's commitment to completing with WHS laws
• Develop at least one WHS Management Plan for your work area
• Conduct WHS risk management for at least one risk with existing risk control procedures
• Conduct required consultative arrangements in the work area
• Ensure that hazard identification is performed during the planning, design, and evaluation stages of at least one organisational change
• Develop and provide WHS induction and training programs
• Measure and evaluate WHS performance against organisation's quality systems framework
• Ensure that work area complies with WHS legislative framework to achieve WHS legal requirements
• Modify WHSMP for areas of improvement
Task 1: WHS Requirements
This task will require you to identify relevant work health and safety (WHS) requirements required to ensure a safe workplace in your work area.
You will use these requirements to develop a WHS management plan for your work area in Workplace Project Task 2.
Work area refers to a specific site within an organisation/workplace that is distinguished by the nature and type of work being undertaken.
This includes WHS requirements from the following sources:
• Organisational policies and procedures
• Organisational WHS training programs
• WHS laws applicable to your state/territory, which must include:
o Legislation
o Regulation
o Codes of practice applicable to your specific work area
Use your workplace/organisation's template to complete this task, or use the WHS Requirements template provided at the Bounce Fitness site.
To complete this task, you must:
• Locate and gather information that you need to ensure a safe workplace for people in your work area from the sources outlined above.
This will require you to access and research information relevant to WHS, including:
o Organisational policies and procedures
o Organisational WHS training programs
o WHS laws applicable to your state/territory, which must include:
• Legislation
• Regulation
• Codes of practice applicable to your specific work area
• Identify the following organisational and legislative WHS requirements in your work health and safety requirements template:
o Consultation and participation requirements, which must include:
• Identifying duty holders
• Identify and approving required resources
• Developing and implementing WHS training programs
o Risk management requirements, which must include:
• Hazard identification
• Risk assessment
• Risk control using hierarchy of control
o Developing and providing WHS training and induction programs

Task 2: WHS Management Plan
This task will require you to develop a WHS management plan for your own work area based on organisational and legislative requirements identified in Workplace Project Task 1.
A WHS management plan is set of plans developed to systematically eliminate the possibility of accidents, illness, injury or fatality in the workplace.
Use your workplace/organisation's template to complete this task or use the WHS Management Plan (WHSMP) template provided at the Bounce Fitness site.
To complete this task, you must:
• Locate and gather information that you need to ensure a safe workplace for people in your work area based on the requirements identified in your Work Health and Safety Requirements from Workplace Project Task 1, this includes:
o Organisational and legislative requirements applicable to your work area
o Organisation's quality system framework
Quality system framework is the conceptual basis used by an organisation to structure processes embedded in management systems.
• Consult with at least two required personnel to identify requirements for your consultation arrangements in the work area.
Required personnel are duty holders in the work area who will be affected by the WHSMP you will develop.
• Develop policy and procedures for your work area on the WHS management planbased on WHS requirements identified in the Work Health and Safety Requirements from Workplace Project Task 1
Your WHSMP must include the following:
o Consultation and participation arrangements, which, at a minimum, must include:
• Identifying duty holders
• Identifying and approving required WHS resources
• Developing and implementing WHS training programs
o Risk management requirements, which, at a minimum, must include:
• Hazard identification
• Risk assessment
• Selecting risk controls using hierarchy of control
• Implementing risk control measures
• Reviewing risks
o Developing and delivering WHS training programs, which, at a minimum, must include:
• Developing and providing a WHS induction program
• Developing and providing WHS training programs

You must also submit evidence of communication/consultation with at least two required personnel of the organisation on their input about the WHS consultation arrangements for your work area.
Evidence must be at least one of the following:
• Email correspondence
• Meeting minutes
• Video recording of the meeting conducted with the stakeholders
Task 3: Approve Resources Required to Implement WHSMP
This task requires you approve financial and human resources required by the WHSMP produced in Workplace Project Task 2
To complete this task, you must secure approval for financial and human resources of your WHSMP submitted for Workplace Project Task 2 following organisational policies and procedures
Review Workplace Project Task 3 - Assessor's Checklist before starting this task. This form outlines the following:
• Resources you are required to access to complete the task. Discuss these resources with your assessor prior to starting this task.
• All criteria your submission must address to satisfactorily complete this task. Your assessor will also discuss with you the criteria outlined in this form prior to the assessment
Submit evidence that you secured approval for the following resources identified in the WHSMP from Workplace Project Task 2 following organisational policies and procedures:
• Financial resources
• Human resources
Evidence must be at least one of the following:
• Email correspondence
• Meeting minutes
• Video recording of the meeting conducted with the stakeholders
Include any supplementary policies and procedures used to approve required resources.
Task 4: Hazard Identification Form and Risk Register
This task will require you to conduct hazard identification and risk assessment on a risk with existing control measures according to procedures developed in the WHSMP submitted during Workplace Project Task 2
Use your workplace/organisation's template to complete this task or use the following templates provided at the Bounce Fitness site:
• Hazard Identification Form
• Risk Register
To complete this task, you must follow procedures outlined in the WHSMP submitted during Workplace Project Task 2 to:
• Conduct consultation with required personnel based on consultation arrangements developed for the following:
o Hazard identification
o Risk Assessment
o Identification of risk controls
Required personnel are duty holders that must be consulted as outlined in the consultation arrangements outlined in your WHSMP.
• Identify inadequacies in existing risk controls based on hierarchy of control measures
• Conduct hazard identification on inadequacies found from existing risk controls
• Assess risk/s of hazards identified
• Select risk control measures to implement based on hierarchy of control measures
• Seek advice from at least one relevant WHS expert on at least two requirements that needs further clarification from your hazard identification, risk assessment or risk control identification processes
WHS experts are individuals or groups who have extensive knowledge and experience in compliance with WHS requirements.
• Identify the following information in your Hazard Identification Form:
o Hazard identified from risk
o Risk associated with the hazard
o Person/people affected by the hazard
o Existing risk control measures
o Initial risk rating
o Further action to be taken
You are to leave the ‘Residual risk rating' section blank. This will be completed in a following task.
• Identify the following information in your Risk Register:
o Description of risk
o Current treatment used to manage risk
o Initial risk rating
o Revised risk control measure
o Resources required to implement risk control measure
You are to leave ‘Actual Date to Implement', ‘Revised Risk Rating' and ‘Date of Review' sections blank. This will be completed in a following task.
Review Workplace Project Task 4 - Assessor's Checklist before starting this task. This form outlines the following:
• Resources you are required to access to complete the task.
• All criteria your submission must address to satisfactorily complete this task.
You assessor will discuss these resources with you, and the criteria outlined in this form prior to this assessment.
Review the template you will use to complete this task. After the assessment, submit the following to your assessor:
• Partially completed hazard identification form
• Partially completed risk register
Additionally, you must also submit evidence of your consultation with the following:
• Required personnel during each of the following:
o Hazard identification
o Risk Assessment
o Identification of risk control measures
• WHS expert to clarify requirements of managing the risks that require further advice
Evidence submitted must be any of the following:
• Email correspondence
• Meeting minutes
• Video recording of meeting conducted with required personnel
Additionally, you must also submit evidence of documentation used as source to identify risks with existing control measures.
Task 5: Implement Risk Control Measures
This task requires you to implement risk control measures identified in Workplace Project Task 4.
The risk control measures implemented must reflect information from the Hazard Identification and Risk Register partially completed in Workplace Project Task 4.
To complete this task, you must follow procedures outlined in the WHS management plan developed in Workplace Project Task 2 to:
• Provide approved resources required to implement risk control measures identified in Workplace Project Task 3
• Implement risk control measures identified in Workplace Project Task 3
• Determine revised risk rating after implementation of risk control measures
• Select risk control measures to implement based on hierarchy of control measures
• Identify the residual risk rating on your Hazard Identification Form
• Identify the following information on your Risk Register:
o Actual date to implement
o Revised risk rating
o Date of review
Review Workplace Project Task 5 - Assessor's Checklist before starting this task. This form outlines the following:
• Resources you are required to access to complete the task.
• All criteria your submission must address to satisfactorily complete this task.
You assessor will discuss these resources with you, and the criteria outlined in this form prior to this assessment.
Review the template you will use to complete this task. If you are using a template from your workplace/organisation, discuss with your assessor to ensure that the template covers all requirements that apply to this task. Otherwise, use the Hazard Identification Form and Risk Register templates provided at the Bounce Fitness site
After the assessment, submit the following to your assessor:
• Completed hazard identification form
• Completed risk register
Additionally, you must provide evidence of the following:
• Provision of resources required to implement risk control measures
• Implementation of each risk control measure outlined in your Risk Register
Task 6: Presentation of Resolution of Risk Management
Your assessor will observe you as youpresent information to required personnel about the resolution of risk management of at least one risk in the work area.
Required personnel refer to duty holders within the work area who were consulted and/or participated in managing the resolved risk.
Information provided can be based on the risk management conducted in Workplace Project Task 3 and Workplace Project Task 4 or any other recently concluded risk management conducted in the work area.
You will be assessed on your practical skills to:
• Present information about consultation and participation outcomes to at least one relevant personnel according to consultation arrangements outlined in the WHS Management Plan completed in Workplace Project Task 2.
If you are providing information based on a risk management process conducted in your organisation, then you must access documents of consultation and participation used throughout the risk management process and documents that provide the outcomes of the risk management.
• Presents information on the resolution of risk management using structure and language suitable for the audience
Review Workplace Project Task 6 - Observation Form before starting this task. This form outlines all the practical skills you need to demonstrate to satisfactorily complete this task. Your assessor will also discuss with you the practical skills outlined in this form prior to the assessment.
IMPORTANT!
If you are providing information on a risk management conducted in your organisation, you must submit copies of documents of consultation with required personnel and results of risk management processes.
Task 7: Include Hazard Identification in Organisational Change
This task will require you toinclude hazard identification during the planning, design and evaluation stages of at least one organisational change made in your work area.
Organisational change refers to any organisation decisions to revise, amend or improve on currently used equipment, processes, work tasks, systems or operations.
To complete this task, you must include hazard identification during the following stages of at least one organisational change made in your work area:
• Planning
• Design
• Evaluation
Review Workplace Project Task 7 - Assessor's Checklist before starting this task. This form outlines the following:
• Resources you are required to access to complete the task.
• All criteria your submission must address to satisfactorily complete this task.
You assessor will discuss these resources with you, and the criteria outlined in this form prior to this assessment.
Submit evidence that hazard identification is implemented during the following stages of at least one workplace change:
• Planning
• Design
• Evaluation
Evidence submitted can include any hazard identification tool or record used in the organisation. This includes:
• Hazard checklists
• Hazard identification form
• Job hazard analysis
Additionally, you must submit documentation of the planning, design and evaluation of the organisational change and any policy and procedures used to complete your hazard identification.
Task 8: WHS Induction Pack
This task will require you todevelop a WHS Induction Pack to deliver to at least one required personnel in your work area.
Information used in the WHS induction pack can be in line with the WHS management plan developed in Workplace Project Task 2 or existing policies and procedures applicable to the work area.
This is to be used to deliver a WHS induction in Workplace Project Task 9.
Use your workplace/organisation's template to complete this task or use the WHS Induction Pack template provided at the Bounce Fitness site.
To complete this task, you must follow procedures outlined in the WHS Management Plan developed in Workplace Project Task 2 to:
• Develop a WHS Induction Pack, which must include the following:
o Roles and responsibilities of duty holders in the work area
o Requirements for managing hazards and risks in the work area, which must include information for the following:
• Hazard identification
• Risk assessment
• Risk control
o Requirements for reporting incidents in the work area
Review Workplace Project Task 8 - Assessor's Checklist before starting this task. This form outlines the following:
• Resources you are required to access to complete the task.
• All criteria your submission must address to satisfactorily complete this task.
You assessor will discuss these resources with you, and the criteria outlined in this form prior to this assessment.
IMPORTANT!
If you are developing an induction program based on existing policies and procedures, you must submit documentation of requirements used to develop an WHS induction pack.
Task 9: Deliver WHS Induction to Required Personnel
Your assessor will observe you as you deliver a WHS induction to at least one required personnel in your work area based on the WHS Induction Pack submitted for Workplace Project Task 8.
Required personnel refer to workers of the organisation who must be familiarised or refamiliarised with the processes and systems of the organisation's WHS systems.
You will be assessed on your practical skills to:
• Deliver a WHS induction to required personnel in a work area based on the WHS Induction Pack submitted for Workplace Project Task 8.
• Provide information about WHS policies and procedures to required personnel
• Present and seek information from others during the induction using structure and language that suits your audience.
Review Workplace Project Task 9 - Observation Form before starting this task. This form outlines all the practical skills you need to demonstrate to satisfactorily complete this task. Your assessor will also discuss with you the practical skills outlined in this form prior to the assessment.
Task 10: Training Needs Analysis
This task will require you to identify and document the WHS training requirements to develop a training program for at least one required personnel.
Required personnel refer individuals working in the organisation who require additional training to fulfil their WHS obligations or advance their personal development.
Use your workplace/organisation's template to complete this task, or use the Training Needs Analysis template provided at the Bounce Fitness site.
To complete this task, you must:
• Identify required training needs based on the procedures outlined in the Work Health and Management Plan (WHSMP) completed during Workplace Project Task 2
• Consult at least one required personnel to determine the following:
o Required WHS training need
Training need refers to the skills, information or capabilities that an individual or group requires to effectively carry out their WHS obligations.
o Determining the training required to address each training need
• Complete a training need analysis based on consultation with required personnel
Review Workplace Project Task 10 - Assessor's Checklist before starting this task. This form outlines the following:
• Resources you are required to access to complete the task. Discuss these resources with your assessor prior to starting this task
• All criteria your submission must address to satisfactorily complete this task. You assessor will also discuss with you the criteria outlined in this form prior to the assessment

Submit the completed training needs analysis to your assessor.
You must also submit evidence of your consultation with required personnel.

Task 11: Provide WHS Training to Required Personnel
This task will require you to provide WHS training to at least one required personnel in your work area.
Required personnel refer to individuals working in the organisation who require additional training to fulfil their WHS obligations or advance their personal development.
Training provided may be in line with the Training Needs Analysis completed during Workplace Project Task 11 or any opportunity to provide WHS training to personnel required in your work area.
To complete this task, you must:
• Access and review the following:
o WHS Management Plan submitted by the learner in Workplace Project Task 2.
If you are providing training based on actual training requirements in your work area, review relevant organisational policies and procedures.
o Training Needs Analysis completed in Workplace Project Task 10
If you are providing training based on actual training requirements in your work area, then you must access the necessary documentation that identifies the training needs or requirements of the required personnel.
• Provide the appropriate training to address training needs of required personnel based on the Training Needs Analysis completed during Workplace Project Task 10.
Training provided must comply with requirements relevant to providing WHS training outlined in the WHS Management Plan submitted during Workplace Project Task 2
If you are providing training based on actual training requirements in your work area, comply with requirements outlined in your policies and procedures.
Review Workplace Project Task 11 - Assessor's Checklist before starting this task. This form outlines the following:
• Resources you are required to access to complete the task. Discuss these resources with your assessor prior to starting this task
• All criteria your submission must address to satisfactorily complete this task. You assessor will also discuss with you the criteria outlined in this form prior to the assessment
Your assessor will discuss these resources with you, and the criteria outlined in this form prior to this assessment.
Submit evidence that you have provided the appropriate WHS training to required personnel.
Evidence can include the following:
• Certificate of completion
• Email of confirmation
• Qualification transcript
IMPORTANT!
If you are providing training based on actual training requirements in your work area, you must submit copies of documents used to identify WSH training needs or requirements of required personnel and policies and procedures used to provide WHS training to required personnel.
Task 12: WHS Evaluation Report
This task will require you to evaluate your Work Health and Safety Management System (WHSMS) applicable to your work area against your organisation's quality systems framework.
Use your workplace/organisation's template to complete this task or use the WHS Evaluation Report template provided at the Bounce Fitness site.
WHSMS can be based on the following:
• WHS Management Plan (WHSMP) completed during Workplace Project Task 2
• Any existing Work Health and Safety System used in your work area other than the WHSMP completed for this assessment
To complete this task, you must:
• Review WHS records of your work area relevant to the following:
o Risk management process, which must include
• Hazard identification
• Risk assessment
• Risk control
o Consultation and participation in resolving WHS issues
o Reports of injuries and illness in the organisation
• Evaluate effectiveness of current WHSMS against key performance indicators (KPIs) in relation to your organisational quality framework.
• Document measures and evaluation of the following areas of your WHSMS in the WHS Evaluation Report:
o Training programs
o Injury and illness performance
o Hazard identification
o Risk assessment
o Risk control
o Consultation arrangements
Review Workplace Project Task 12 - Assessor's Checklist before starting this task. This form outlines the following:
• Resources you are required to access to complete the task. Discuss these resources with your assessor prior to starting this task
• All criteria your submission must address to satisfactorily complete this task. You assessor will also discuss with you the criteria outlined in this form prior to the assessment
Your assessor will discuss these resources with you, and the criteria outlined in this form prior to this assessment. Include any supplementary documents/sources used to evaluate your WHSMS.
Review the template you will use to complete this task. If you are using a template from your workplace/organisation, discuss with your assessor to ensure that the template covers all requirements that apply to this task. Submit the completed WHS evaluation report to your assessor. Include any organisational WHS records used measure and evaluate the following:
• Training programs
• Injury and illness performance
• Hazard identification
• Risk assessment
• Risk control
• Consultation arrangements
Submitted evidence must correspond to the tools used to record decisions as outlined in the WHSMP submitted in Workplace Project Task 2 or any corresponding WHSMS used by the learner.
IMPORTANT!
If you are using an existing WHSMS in your work area, you must provide copies of documentation where you recorded the necessary management systems.
Task 13: Modify WHS Management Plan
This task requires you to modify at least two areas of improvement in the WHSMP used in your work area.
Prior to commencing this task, you must:
• Have completed Workplace Project Task 2; or
• Have access to your work area's WHSMS other than the WHSMP developed for this assessment.
This must be the WHSMS accessed to complete Workplace Project Task 12.
To complete this task, you must:
• Access and review the WHS Evaluation Report completed during Workplace Project Task 12
• Identify at least two areas of improvement in the WHSMP.
o If you're referring to the WHSMP from this project assessment, you can base your areas of improvement on the evaluation conducted in Workplace Project Task 11
o If you're referring to the WHSMS from your own work area you can base your areas of improvement on any of the following:
• Evaluation conducted during Workplace Project Task 11
• Any other areas you find from the WHSMS that require modification
• Modify the section of your WHSMP or WHSMS document based on your findings. Outline the modifications done using the version control section of WHSMP document.
