Established Patient History and Physical Note: HISTORY: Miguel came to the clinic today with C/O cough. The cough has been present for months and does not seem to involve an illness. He did have a cold a few weeks ago with severe nasal congestion and sore throat. He thinks the cough was present before and after the cold. He also noted having some problems with ear popping and fullness. He has tried OTC cough preparations without much success. Denies any previous history of sinus surgery or nasal injury. He notes he has a history of nasal congestion, coughing, and habitual throat clearing. He denies any difficulty breathing or problems with wheezing. He believes he has some degree of allergy symptoms on a seasonal basis, but he is not sure what he is allergic to. PAST MEDICAL HISTORY: Miguel is a recent immigrant from Mexico and a new father. He recalls having frequent colds as a child in Mexico and his mother giving him eucalyptus steam baths to help his congestion. PAST SURGICAL HISTORY: No surgeries or hospitalizations. CURRENT MEDICATIONS: None ALLERGIES: NKDA SOCIAL HISTORY: Miguel and his family live in an older trailer.
