Case Study Scenario:
Mr Keith Davidson is a 63-year-old man. He has been admitted to Holmesville Hospital 74 days prior with Acute Pancreatitis.
Keith lives with his partner in a rental property with his wife. He is currently on disability income support with his wife working casually.
David has a past history of
• Obesity (BMI 46)
• Type 2 Diabetes requiring insulin
• Peripheral Neuropathy
• Retinopathy
• Recent below Knee Amputation of the right leg
Keith's hospital stay had been complicated with multiple inpatient falls and non-compliance with care direction had resulted in 1:1 direct supervised care throughout the admission.
For management of his conditions, Keith is prescribed the following medications;
Novomix 30, Targin, Metformin, Gabapentin, and Vitamin D.
Allocated to Keith's care, you start to establish the rehabilitation care plan with the treating team.
