Working on a database and have put together the ER and the Logical Diagram.

Need assistance with putting together actual table using either SQL Server 2012 or 2016 - Preferably 2012 edition- to create the DDL statements for all 7 of the tables.

Attached is the report, ER and logical diagram describing the table.
