Question

EMS from home after sudden onset of right-sided weakness and difficulty speaking that started 1 hour prior to arrival. On examination, the patient has global aphasia, left gaze preference, right hemianopsia, right facial droop, right hemiplegia, and dysarthria. A stroke alert was activated. You perform the NIH Stroke Scale, and his score is 22. The patient was immediately taken for a CT scan- CT angiography showed a left middle cerebral artery occlusion. Patient smokes 1 ppd/30 years, no alcohol or illicit drug use. He is a computer engineer and sits in front of a computer all day.
