Kaveh is a 40-year-old man who has recently moved interstate to be closer to his mother who has recently been diagnosed with cancer and is in palliative care. Kaveh was diagnosed with depression at an early age and was frequently using the formal services in his old town. As part of his support strategy Kaveh is going to continue to use the available state-wide counselling and support telephone helpline services that he usually accesses. Kaveh's previous case manager is unable to continuing supporting him as that service was provided through a small local agency and interstate care falls outside the eligibility criteria for their service. Kaveh's previous case manager sent a referral to a local mental health care coordination service in Kaveh's mother's town. His new care coordinator is Emma and she has started working with Kaveh to identify which local and state formal services he would benefit from.
Kaveh's new town is smaller than his old one and doesn't have all of the support services that he use to access as part of his support plan, like a case manager and mental health nurse. Kaveh particularly notices the lack of public transport and employment options. Emma has explained to Kaveh that the important thing is to identify his unmet needs and difficulties with the new services so that they can collaborate with other services in order to better cater for his mental health needs.
Emma's role as a care coordinator differs from Kaveh's previous case manager and she often has to explain her role and the roles of the team with regards to limits of their abilities and the need for referral onto specialty services.
Emma has found working with Kaveh to be quite challenging. She discusses this with her supervisor Anne who encourages her to reflect on her practice and determine if there is a way that she could improve on the service provision for similar situations in the future.
