Element2 : Apply Root Cause Analysis in the Incident Investigation
2.1Analyse the event sequences of the incident by using the Root Cause Analysis (RCA) method.
2.2 Develop deviations analysis to identify the causal factors of the accident.
2.3 Establish the Root Cause Analysis (RCA) by identifying and analysing the causes of an incident

Element 3 : Apply analytical risk management techniques in Incident Investigation
3.1Describe hazards associated with an Incident and the control measures that should have been in place to prevent it.
3.2Conduct a management change analysis process for the incidents and develop a simple change analysis worksheet.
3.3Apply the barrier model concept and change analysis in the example scenario / incident.

Element 4: Apply specialized analytical techniques in the Incident Investigation
4.1 Conduct Ishikawa's Fishbone Analysis to assess the relative impact of different failures.
4.2 Apply the principle of "5 Whys" for investigation of incidents.
4.3 Analyse incidents by using Ishikawa's model for final outcome of Root Cause Analysis (RCA).
4.4 Produce a Root Cause Analysis (RCA) map of the example/scenario of incident.

Scenario: Warehouse Incident

Task 1:
A) Outline 5 casual factors for above scenario with level of accountability i.e. Low or High.

In Casual Factor column put the reasons that caused this accident, for example, there is no training or checking or there is no barrier + put who is responsible for these things then marked as low or high)
B) Explain the event sequences of an incident from above scenario by using the event and casual (Draw same picture below and write in the squares the events and in the circle the casual that happened from above scenario above and in the end put ambulance).
C) Outline the primary root causes of the incident using anyone of the analytical techniques.

Task 2: Answer the following question with reference to thescenario(5 marks per ques-tion)
A) Describe any fivehazards mentioned in the scenario and give acontrol measurefor each of them.
B) Develop a change analysis worksheet(attached)by conducting a management change analysis process for the incidents mentioned in the scenario.
C) Complete a barrier analysis via a table by using the information given in the scenario.

Task 3 Answer the following question with reference to the scenario(5 marks per ques-tion)
A) Explain the procedure for drawing fishbone analysis diagram.

B) Identify the root cause of the accidentusing "5 Whys" technique.
(need to write similar about 5 why's from scenario above but be sure to add more in answers in details not briefly)
C) Draw Ishikawa's Fishbone Analysis to assess the relative impact of different failures.
D) Present the Root Cause Analysis (RCA) map using Ishikawa's Fishbone Analysis and "5 Whys" model.
