Question

Your client needs to take a medication in patch form. What can you do to assist? Cut the patch into smaller pieces for easier application and to reduce skin irritation. Help the client avoid applying to sensitive or easily irritated skin, apply the patch yourself, and assist the client with good hand hygiene afterward to prevent the medication from getting into the client's eyes or mouth. Wear gloves to avoid exposure to the medication, open the package, and help the client to find an appropriate site to apply it. Assist the client to a comfortable position in bed or on the toilet and apply the patch yourself.
