Question

The client is alert and orientated x4, but anxious. The client appears dyspneic when moving from the wheelchair to the bed. The client appears pale and states they are "just so tired and this is all so scary, am I going to be okay?" S3 gallop noted while listening to heart sounds. Crackles heard throughout bilateral lungs. Generalized edema present throughout. Bilateral radial pulses 1 , bilateral pedal pulses auscultated with doppler. Bilateral lower extremities pale and cool. Bowel sounds are normoactive in the RLQ. Current vital signs are a temperature of 36.9°C (98.4°F), pulse 112/min, respirations 28/min, blood pressure 186/78 mm Hg and SpO2 of 92% on RA. BNP 535 ng/L.
