SMK Bersatu would like to keep track of all its students' co-curriculum activities and be able to produce the certificate to its students at the end of their school year. You have been asked to design a database for the school. In designing the database, you have to consider the following criteria:

• Students can be involved in as many co-curriculum activities as they want and some students may not be involved at all. Information about a student would be birth certificate number, name, address and age. Other information to be kept is the year the student was involved in the activity and the position held.
• Information about co-curriculum activities would be the activity code, activity name, venue, date of the activity and category. Many activities are held for each category. Category information is kept separately, where the information will be category code, description of the category and the weightage. The weightage is used to calculate the merit for the co-curriculum activities involved.
• The school teachers are in charge of co-curriculum activities. Information about the teacher would be the employee number, teacher's name and years of experience. Each teacher can be responsible for only one activity.
• A teacher conducts classes. The class code, class name and the duration of each class held by each teacher are stored.

a) Draw the entity relationship diagram (ERD) by using Crow's Foot notation that shows all entities, attributes, relationships, relationship name, cardinalities, primary keys and foreign keys.

b) Explain ONE (1) example of report that can be generated from ERD in (a).
