Assignment:

Re-word using action and dynamic words. I have received a form for conservatorship from a transfer patient from Milan. THere is family dynamic issues between the brother and sister. THe sister currently has power of attorney and is trying to get conservatorship. There was a previous consevatorship clompleted by the former physician but it appears the pcg is trying to still get the consevatorship granted by using the hospice diagnosis to get it approved. We do not have aenough medical history to complete the attached. Does the medical director typically sign off on the attached form? Do you know of any other resources or options for the pcg?
