Problem

The CDC has funded programs such as WISEWOMAN which provides education and tools for uninsured, low income and under-insured woman over the age of 40 who are at risk for heart disease (CDC 2023). Programs such as these provide opportunities for clinicians such as community health nurses the ability to improve outcomes through outreach and education. The fortunate role of the community health nurse is that they are in close contact with the communities and have direct access to the individuals who can share their stories about their needs for fair and equitable health care. Community health nurses are critical in creating innovative ways to deliver healthcare and create opportunities to help bridge these gaps. Innovators such as Florence Nightingale and Mary Breckinridge were some of the first to develop ways to improve health care delivery among the community. Does anyone know of any other community programs, either federal or local, that has been beneficial in helping bridge the gap between lower income/at risk populations and their access to equitable healthcare?
