Dorothy Calkins is a 24-year-old waitress who comes in to see her doctor because her long standing anxiety problems, which she has been self-treating with cannabis, have reappeared along with new symptoms of depression. She reports generalized anxiety, crying spells, poor sleep, a 10-pound weight loss, mild abdominal pain, and headaches. She has smoked cannabis 1 or 2 times daily for the last 6 years and has not been able to go without cannabis for more than 2-3 days because her anxiety symptoms get worse when she stops. Ten days ago, however, she decided to stop because she is getting a new challenging job that requires her to focus carefully for extended periods of time and to multitask. Moreover, the job also requires urine testing for drugs. Over the last week or so, she has been "grumpy" and has had fights with her boyfriend, plus she has been "fidgety".
