Mr. Jones is a 51-year old gentleman seen at the request of Dr. Smith for evaluation of his cardiac status. He has underlying ischemic cardiomyopathy first diagnosed in 1999. Since then, he has done well but was recently seen and evaluated by Dr. Smart, Thoracic surgeon, following a diagnosis of a lung nodule that was felt to be due to possible underlying cancer of the lung. He was admitted in October with an acute myocardial infarction and was noticed to have hemoptysis (coughing up of blood) and a nodule. He has long-standing significant coronary disease that has been documented by cardiac catheterization.
Cardiac Risk Factors Include:
1. Hypertension
2. Smoker; half a pack a day
3. Occasional alcohol; used to drink up to two cases of beer a week
4. No diabetes
5. Elevated cholesterol for at least 10 years
6. Socially inactive
7. Father had heart disease and died at 48; brother had a heart attack in his 50s
