You are a Nurse Practitioner working in the local urgent care center. Mike, a 26 year old male with a history or IV drug use, presents with a fever, dyspnea on exertion and an abscess on his left forearm. On physical exam, you note that he has crackles in the bilateral bases of his lungs and he has a 3/6 diastolic murmur at the left lower sternal border. Explain other physical findings that you might expect to see in this patient. What will his work up include and discuss the likely diagnosis and other differential diagnoses.
