Problem

There was a nurse leader who was legally held accountable for being negligent in their supervision and task delegation to ensure patient safety. The unit had a patient taking remodulin for pulmonary hypertension, and the nurse leader assigned this task to a new graduate who was about one month into residency without giving instructions on the drug. The nurse had no idea about the medication or the importance of having it run throughout. This requires the nurse on duty to have an extra bag and pump ready to infuse the medication as soon as the other one ends, making it a continuous process. The previous nurse did not leave an extra bag which was required, and the nurse leader did not bother to confirm if the new nurse had the medication in hand and if they knew what to do. When the medication ended, everyone was scrambling to get the pharmacist to prepare the medication as soon as possible, and there was a rapid response team waiting outside the door.
