Please note, for this assignment we will not be using the case plan form provided in your text. That will come later.

Instructions
1. Decide whether you want to assess an adult or juvenile case.
2, Access the appropriate risk assessment tool (see below) and familiarize yourself with the format and scoring procedures. Please note, that in normal circumstances the use and scoring of this instrument requires a detailed scoring guide, intensive training, and certification. Here, I am just wanting to give you a sense of how it works. Make your best effort to score the instrument based on the information within the instrument itself. Here is a very brief instructional video on how to score the instrument. Please note, in the video I make reference to your case plan; you are not preparing a case plan this time around. So, the information you gain from the assessment will be used to develop the 2nd required paragraph rather than a case plan.
3. Watch/listen to the assessment video. I recommend that you avoid trying to score the instrument as you are listening to the interview. Take detailed notes and score it afterwards.
4. If you selected an adult case, click on the links below to access the risk assessment tool, the assessment interview, and a self-report and file review form that will help you score some items in the instrument. Please note, that the client has already been sentenced to probation. The assessment will be used to guide decisions made about the intensity and type of supervision and treatment.
