Assignment: Interprofessional Collaborative Project for Online Students Maternal Health Case Study

Purpose: The purpose of this Common Intellectual Experience is to apply concepts from pharmacology and disease pathology along with health assessment techniques and promotion strategies to enhance care for the client in the case study. Principles of law and ethics along with interprofessional collaboration will also be explored.
Patient/Client: Terry is a42 yo Gravida 2 Para 0 (G2P0) African-American female presenting to a walk-in clinic setting reporting lower abdominal pain and cramping that has been increasing over the past 12 hours along with some spotting. She is currently 14 weeks pregnant. She has had two miscarriages previously and is concerned she is losing this baby as well. Terry states she came to the walk-in clinic today because she lost her insurance and can't see her regular OB. She was told this clinic was a "free clinic."
Her prenatal history has been negative thus far. Prenatal vitamins were initiated at 7 weeks and prenatal care at 8 weeks. All prenatal labs, Panorama screen, NT ultrasounds have been within normal limits. Client is alone, but is frequently texting her "spouse". Client identifies as "non-binary" and refuses to check the box as male or female on the check-in form. Appears tearful when called back from the waiting room.
1. Past Medical History: Two previous miscarriages at 12 weeks and 14 weeks. Last miscarriage was 2 years ago.
2. Past Surgical History: Appendectomy at 22 years of age
3. Medications: Prenatal Vitamin (PNV); St. John's Wort 900 mg
4. Social History: Client reports she has been in a relationship for 2 years with her partner, Kia. who is referred to as "wife". Reports a previous marriage where "he left" after the two miscarriages. Client works full-time as a data systems analyst. Client expresses a lack of social support as her family of origin live in a different state. Client reports past negative experiences when interacting with members of the healthcare community as they do not honor her "preferred pronouns of they, them, their."
There is no disclosed history of a personal substance misuse. The client did drink socially prior to finding out about the current pregnancy. She expresses concern about the potential for postpartum depression after delivery, as their mother experienced severe PPD after each of her pregnancies.
5. Family Medical History: Paternal hypertension; maternal breast cancer; Terry also reports depression "runs strong" on the maternal side of the family.
Review of Systems

• HEENT: (-) bilateral ear pain (-) ear drainage; (-) recent cold (-) eye drainage, discharge, sore throat; (-) rhinorrhea or nose bleeds
• Cardiac: (-) heart palpitations or history of cardiac issues
• Respiratory: (-) asthma (-) cough
• GI: (-) nausea, (-) vomiting, (-) diarrhea, (+) abdominal cramping and pain 4/6
• GU: (-) dysuria, frequent urination
• MS: (-) for any musculoskeletal issues
• Neuro: (-) history of seizures or other neurological conditions
• Skin: (-) rash
• Allergies: none reported
• Psych: (-) mental health history
• GYN: currently 14 weeks pregnant; (+) spotting and abdominal cramping
Physical Examination:
• Vitals: Ht: 5'3" Wt: 155 lbs; BP 136/94 P 68 Resp rate 18 Temp 98.0
• HEENT: Head is normocephalic. The sinuses are nontender. Pupils are equal and reactive. The nares are patent. The oropharynx is clear without lesions.
• NECK: Supple without lymphadenopathy. Thyroid normal size, without nodules.
• HEART: S1, S2 heard. Regular rate and rhythm.
• LUNGS: CTA, no crackles or wheezes
• ABDOMEN: Soft, nontender with good bowel sounds heard.
• EXTREMITIES: Without cyanosis or clubbing. Mild peripheral edema noted in feet. +2 Deep Tendon Reflexes (DTR).
• NEUROLOGICAL: Oriented to time, place, person, situation.
• Skin: Warm, dry, intact without any rash. Appendectomy scar noted. Stretch marks noted across the abdomen.

I. Pharmacology:

What medications would you expect to be ordered for this patient? List the medications and describe potential side effects for the patient from these medications. What is your role as the nurse in administering these medications? What other forms of treatment/medical management do you anticipate being done for this patient?
