Question: Minor correction on a medication review assignment based on the following professor's comments:

This revision is better but needs more of a detailed management plan for each clinical condition.
You have advised to continue the use of fluoxetine, although this has a major interaction with tamoxifen and is advised to avoid.
Also, her test results show that her hypertension has been controlled for some time now. This can be seen in her annual review ( done 4 months ago). All in the GP notes. Therefore would it not be Better to take her off indapamjde completely and monitor.

Also need to discuss why she hasn't been offered a calcium channel blocker and why they've gone straight to giving her third step treatment which is a thiazide like diuretic

Opioid abuse has not been discussed. She has been taking co-codamol for 3 years now and there has been no review done. Long term opioid use is not recommended. You've not stated the need to explore this and take her off opioids by tapering down and managing her pain with an alternative.
