SCENARIO 1: Jane is a 68-year-old Aboriginal woman who grew up in a remote community in the Northern Territory. Jane was taken away from her parents when she was 3 years old and raised in a mission. Despite being told she was an Aboriginal Australian, she had little knowledge about her culture and traditions.
When Jane was 18, she moved to the city for work. In the two years she was employed at the organisation, she experienced racial discrimination from her manager and work colleagues who made derogatory comments about Aboriginal Australians. Jane felt isolated and disconnected from her identity and culture.
Jane struggles with depression and anxiety, finding it difficult to trust people. She avoids situations where she might experience discrimination and often feels like she doesn't belong in the broader community. Jane found comfort in reconnecting with her culture and traditions, finding a sense of belonging. The damage caused by her experiences left a deep wound that has affected her relationships and mental health.
Jane has been admitted to hospital 3 days ago with pneumonia. You are the Enrolled Nurse caring for Jane this shift.
1. As an Enrolled Nurse is important that you are aware of your own culture and understand how it affects your relationships with people from other cultures. Reflect on your personal experiences and knowledge related to cultural awareness.
a. How do you believe your cultural awareness, or lack of, might influence your interactions and engagement with Jane?
b. Discuss two (2) strategies you would employ to enhance your cultural competence and ensure culturally safe and effective service engagement for Aboriginal and/or Torres Strait Islander peoples.
c. Discuss two (2) ways western systems and structures impact on Aboriginal and/or Torres Strait Islander peoples and their engagement with services.
2. Describe how Jane's traumatic experience may impact on her ability to:
a. Make health decisions
b. Communicate with others
c. Understand and retain information
3. Jane has previously been unable to get out of bed due to shortness of breath. As her condition is improving you discuss taking her to the shower. Jane refuses a shower; you reassure her that you are able to assist and it will make her feel better. On discussion with Jane she discloses that a male nurse had attended to her bed bath when unwell. Jane was very distressed by this situation due to her cultural beliefs about Women's Business.
a. Based on Jane's feedback, identify two strategies that can be implemented to improve cultural safety.
b. Where would you document Jane's cultural preferences to ensure that this situation doesn't reoccur?
4. Everyone has cultural biases and their own cultural lens from which they view the world. Reflecting on your biases requires you to be open to the possibility, to learn new things about people and their culture and to be ready to acknowledge that some things you have learned are incorrect. It is important to reflect on your own cultural bias and to think carefully about the ideas and beliefs that you hold about other cultural groups.
a. Please reflect and provide a brief overview of the cultural biases or misconceptions that you had prior to beginning this unit of study.
5. Cultural safety is a philosophy used to guide the thinking and actions of health professionals. It promotes respect for cultural uniqueness and values the importance of these in caring for others.
Legislation and industry standards are intertwined with the concept of cultural safety with codes, standards and policies turning legislation into behavioural requirements.
a. List two (2) current legislative acts which support Aboriginal and Torres Strait Islander peoples cultural safety in a health care environment.
b. Review the Nursing and Midwifery Board's code of conduct for nurses. (this link will download document). Explain how this code directly supports cultural safety.
c. State two (2) ways health care providers can evaluate whether these policies are effective.
SCENARIO 2: You are an Enrolled Nurse working in a rural clinic in the far North Queensland. You have noticed an increase in patients receiving treatment for rheumatic heart disease. On researching the condition you discover that rheumatic heart disease is preventable and disproportionately affects Aboriginal and/or Torres Strait Islander peoples living in regional and remote areas. You discuss your findings with the clinic manager and propose developing community education sessions about preventable health conditions directly affecting the community.
1. Your manager is supportive of the idea and wants you to think about a way to encourage self-determination and community control of the program at your clinic.
