1. Ms. Pertrivuti presents complaining of difficulty walking owing to severe osteoarthritis in the hip. A problem-focused history and examination was performed with straightforward decision making. I prescribed medication to alleviate the pain and supplied her with a cane.

2. An established patient presents complaining of a sore throat for 4 days. A problem-focused evaluation and management service was performed, with automated CBC and automated differential WBC count. Antibiotics were prescribed for strep throat. The patient will be seen in 10 days for a follow-up.

3. The patient presents to the Emergency Department with a 4.5-cm laceration on the left leg. A Level II evaluation and management service was performed. Debridement and complex repair of the wound was done. A disposable drug delivery system, at a flow rate of 5 mL per hour, was utilized.

4. Mr. Smith underwent an arthroplasty of the knee, condyle, and plateau, medical compartment for aseptic necrosis of the bone. The procedure was performed in the Ambulatory Surgery Suite. The patient tolerated the procedure well. Discharge instructions indicate non-weight-bearing activity for 6 weeks. The patient was provided with a pair of crutches (adjustable with tips and handgrips).

5. Mrs. Hartenshogan arrived today for a scheduled colonoscopy. The patient has been experiencing severe abdominal pain and nausea after eating. A flexible scope inserted for visualization proximal to the splenic flexure revealed inflammation and infection of the peritoneal cavity. Sacs of mucous membrane are protruding through the muscular wall of the colon. The scope was withdrawn and the patient was moved to recovery.

6. Mr. Fordmanston was admitted to the hospital with coronary atherosclerosis of native arteries, for a coronary bypass. The patient also has malignant hypertension. Three coronary vessels were bypassed during the procedure. The patient tolerated the procedure well.

7. A patient with a malignant neoplasm of the ureter was admitted to the hospital for an ureterectomy. A partial ureterectomy was performed and the formation of other cutaneous ureterostomy. The patient tolerated the procedure well.
