Case Study: Fred is 86 years old and has congestive heart failure. He has been hospitalized several times over the past two years and his daughter whom he lives with tells you in confidence that she can no longer leave him alone while she works. According to his daughter he gets up several times a night due to shortness of breath and the need to urinate. She worries that he may fall. You note that he is dyspneic with moving from bed to chair and his voice is weak. He has lost 15 lbs. in the last month and suffers from diarrhea 6-7 times a day.
