There are many stakeholders and interests when it comes to health and health care. Three of the main stakeholders are providers, patients, and the health care organization in which care is delivered and received. The IOM's six dimensions of quality provide a useful framework to pursuing high value health care. However, sometimes stakeholder interests will conflict across some of the quality dimensions, creating barriers to achieving a high quality, high value health care system.

This exercise asks you to identify some of these potential barriers from the perspective of each of the primary stakeholders.

  1. Consider the six STEEEP quality dimensions for health services promoted by the IOM (Safe, Timely, Efficient, Effective, Equitable, Patient Centered). Select the one dimension you believe is most important to achieving high value health services.
  2. Consider three major stakeholder groups: providers, patients, healthcare organization. How does your selected quality dimension impact each of the three stakeholder groups?
  3. Now consider the possible barriers to achieving high value health services, as they relate to the quality dimensions. Examples are such things as unrealistic expectations of the healthcare system, resource limitations, existing work processes (how things are currently being done), financial considerations, organizational inertia, entrenched attitudes, and resistance to change. Given the quality dimension selected at #1 above, identify two barriers most likely to prevent stakeholders from achieving the quality objective.
