Mary is a 48-year-old Indigenous Australian who lives with her extended family within a community in East Gippsland. She has four (4) grown children and six (6) Grandchildren. Mary is well known and respected locally as she cares for the elderly and teenagers within her community.

Mary rarely visits her doctor but has become extremely tired and is suffering from some shortness of breath, headaches and some dizziness, so has decided to make an appointment with her local GP. Mary smokes 25 cigarettes per day and consumes (2) cans of beer per week. Both of Mary's parents died of heart-related illnesses in their 50's.

Objective data:

Mary's physical examination reveals the following data:

BP 192/103 mmHg (previously recorded BP 186/94mmHg 18 months prior)
Pulse 96 bpm, weak in strength
Respiratory rate 24 per minute at rest, breathless on exertion (walking into the consulting room)
SaO2 92% in room air
Height 145cm
Weight 86Kg

The Doctor investigated further and ordered the following diagnostic tests which revealed the following:

ECG- sinus tachycardia
Cholesterol 12mmol/L (normal<4mmol/L)
HbA1c 13.6 (normal <8)
Serum Troponin I -0.4ng/ml (normal 0.1-3.1.ng/ml)
