Simulation based assessment task 1:
Assume that you are working as a community nurse under the supervision of a registered nurse. Your organisation has organised an information session to people who have migrated to Australia from different parts of the world as part of community empowerment and health promotion.
Mr Vinh Chang, 45 years old, migrated from Vietnam to Australia five (5) years ago. He got his permanent residency in Australia four months ago. Mr Chang has five young children, under the age of
12. His children live with him. He is separated from his wife.
Mr Chang is working as an asbestos removalist and a plumber who installs and repairs water, drainage, gas and sewerage pipes and systems. He lives in a rural town in South Australia. He has recently moved to a different home.
Mr Chang is a chronic smoker and alcoholic. He is obese and recently started having breathing difficulties. He is upset about his health now and he wants to live a normal life for his children.
Mr Chang participated in an information session organised by the community nurses about health promotion. Your supervising registered nurse asked you to provide required information and support to Mr Chang.
As part of Task 1, perform the following activities:
a) Collect basic information about Mr Chang's health status through appropriate interview questions and discussion. The information must include:
• Risk factors associated with the current health status, e.g. family history and medical history.
• Social circumstances applying to the current health status, e.g. work pattern, socio- cultural, economic factors influencing health.
• Emotional wellbeing of the person.
b) Provide information on the health professionals Mr Chang should approach to seek further assistance from in relation to his health status. Discuss the roles of these health professionals.
c) Identify the health agencies and services Mr Chang could approach to address his health requirements. You are expected to apply a knowledge of levels of health care when demonstrating this activity. You must also demonstrate how you would assist Mr Chang in accessing one (1) of these agencies.
d) Discuss appropriate health care service funding sources and health-related insurances Mr Chang could apply for or access.
e) Identify two (2) available community health promotional strategies that Mr Chang could access to address his health needs. Discuss the effectiveness of these community health promotional strategies with Mr Chang.
Simulation based assessment task 2:
Assume that you are working as an enrolled nurse (casual) in EQUALS' Hospital.
A patient from a Chinese background was admitted in your ward this morning. You completed the physical health examination and health history of the person as requested by the registered nurse.
The summary of the health examination and history collection is provided below:
• The person has difficulty in understanding English due to a culturally and linguistically diverse background.
• The person is confused whether they can undergo acupuncture along with the newly prescribed medications.
• The person has had reduced food and fluid intake for a week.
• The person is weak, anxious and worried about their health.
• The person has high blood pressure and is not on any medicines.
You are required to identify four (4) different healthcare personnel you should liaise with to provide comprehensive care to your client. Discuss with the assessor the role of these healthcare personnel in addressing the healthcare needs of the person.
Demonstrate to your assessor how you would establish collaborative relationships with these healthcare personnel. You must demonstrate how you would actually interact with these personnel under supervision rather than telling your assessor "I will call them or I will page them".
Simulation based assessment task 3:
Your organisation is preparing for an accreditation audit. You are coordinating an interdisciplinary team meeting as requested by the registered nurse to discuss the progress of preparations for the audit. Your team includes a total of five (5) team members including yourself.
All your team members have been active and contributed effectively to audit preparations. However, you noticed today that Cheryl, a team member, was extremely quiet. She sat with folded arms. Sandra, another team member, was noticed occasionally staring at Cheryl. Sandra, also, was silent most of the time.
Demonstrate the best approach in this scenario to achieving effective team work and supportive group dynamics when working in an interdisciplinary team.
Demonstrate how you would liaise with Cheryl and Sandra in supporting them.
