Identify a reliable newspaper or magazine article or any other reliable source describing a health and safety accident that occurred within Ontario in the last years. The accident must be relevant to an industrial establishment and the provincial legislation. It requires you to apply health and safety concepts to analyze a real-life accident so it may be prevented in the future.

1. Provide an introduction and a complete summary of what occurred, the facts describing the accident, and the outcome. It is recommended you pick a case where the investigation has been completed.

2. Discuss the relevant regulations and any other legislation applicable to the accident and how it applies to the specific industry or sector. Consider how the language might indicate different interpretations of the legislation or is it clear.

3. Analyze the accident and discuss how it might have been prevented. Consider human, environment, materials, equipment factors, and the process involved. For example, when considering human factors or people involved, what did the worker(s) do or not do in this case. Your analysis must consider all aspects of the incident. Consider how the loss to human beings and/or property could have been mitigated?

4. Discuss how your findings (discussed above) are relevant to the practice of HR. For example, is there a policy and /or procedure that could have prevented the accident. What areas of HRM and/ or HR policies are relevant to or impacted by this accident?
