ROS: She received all of the routine childhood immunizations, a complete hepatitis B series, last tetanus shot was at 5yoa. Family history no medical problems, the exception was the mother stated was their 21-year-old cousin died of sudden death. On exam: vital signs and weight wnl for age group. ENT, Lungs, GI, Skin, Neuro: Negative findings; Cardiac: on auscultation a 2/6 systolic murmur is present; on valsalva it increases in intensity. Peripheral pulses are strong and symmetric; capillary refill within 3 seconds, without cyanosis or clubbing in all extremities.
