Problem

A mother brings a 3-year old son to the pediatrician because of persistent, frequent nosebleeds during the past month. The nosebleeds occur twice a week and continue for 2 hours before stopping. On physical exam, the child shows petechiae and bruising on the skin. Analysis of the stool is positive for blood. CBC shows normally formed RBCs and WBCs. The platelet count is 25,000 platelets/mL (abnormally low), and hematocrit is low. Partial thromboplastin time and prothrombin times are both normal. The patient is started on a short treatment of prednisone, and the symptoms disappear after 3 weeks. Please answer the following questions:

I. Explain the physiologic mechanisms by which healthy endothelial cells normally prevent platelet aggregation.

II. Discuss the different kinds of cell surface receptors present on platelets and identify to what these platelet receptors can adhere.
