The patient has not attempted any weight loss during the period. The patient claims compliance with exercise therapy. Upon exam, the patient has no venous stasis ulcer. The patient's left calf has extensive twisted, enlarged veins that appear dark purple. Patient denies pain, aching, cramping, burning, itching, and claudication. Current blood pressure is 120/80. After lengthy discussion, the patient wishes to proceed with surgical sclerotherapy (e.g. CPT code 36470). The patient is Medicare primary and Anthem BC/BS secondary.
