Introduction - Patient safety, defined by the World Health Organization as "the prevention of errors and adverse effects to patients associated with health care," has become a priority in health care settings. Most of the research and focus in this field so far has focused on hospital settings. However, the vast majority of health care consultations actually take place in primary care.

Think about a time when you, your relative, or your friend was a patient in a hospital or other health care setting.
Discuss the following -
How safe it felt in your perception. On a scale from 0% to 100%, how would you rate the degree of safety/security that you felt?
The factors that contributed to your perception of safety.
The specific clinical safety practices that you noticed (Use the tables from the Office of Inspector General article to see a list of possible adverse events (Levinson, 2010, p.16, p. 21).
List at least three initiatives that could have been done to improve your perception of safety. Support the rationale for these suggested initiatives by scholarly sources.
