Problem II. Discuss one error or potential adverse event in the discharge planning process. Discuss some of the negative outcomes that can occur because of that error. It can be anything from a lack of education provided to the patient regarding how to take their medication, or inefficient planning with the patient's family/caregiver, making the patient remain in the hospital bed several hours longer.
