Sw has attempted to reach pcg to discuss medical directors order for labs. PCG Has not return phone call or messages. Home Health Aide notified pcg of medical director request and that SW will be droopping off the order for pt to go to Baptist TIpton to get labs drawn. Home Health Aide reported pt stated it was okay for SW to drop off order. Sw called on Tuesday, May 28 to drop off order but no response. Sw attempted to drop off order today, May29, 2024 no answer to phone call and no answer at home . S placed in envelope and place in family mailbox and left message informiing pcg that order was in mailbox. SW also informed Home Health Aide who stated she would conveyinformation to pcg when she going for her next visit on 5/30/204. no additional info noted.
