Mrs Rochelle Barnes is an 88-year-old First Nations woman who has recently been admitted to a residential aged care facility (RACF) where you are an enrolled nurse. The RACF is staffed with a higher ratio of Personal Care Workers (PCW's) than Enrolled Nurses, and there is currently no Indigenous Liaison Officer, or community referral point for First Nations residents.
Mrs Barnes was admitted to the RACF for safe care for her deteriorating dementia diagnosis, and the ongoing hemiplegia (left sided) from a CVA 2 years ago.
Mrs Barnes requires assistance with all ADL's, ensuring at all times dignity of risk is maintained.
Mrs Barnes has been found on the floor in the shower, her shower chair did not have brakes on, and she has fallen when trying to get up to get 'warm' as she was 'freezing in the water'. The team leader tells you not to worry about getting a review from a medical officer as Mrs Barnes is 'old, has dementia, and is likely to have little treatment initiated due to her age anyway'. Upon asking about what happened, you find out that Mrs Barnes was getting 'agitated' when the male PCW was trying to shower her. Mrs Barnes' family had requested for personal cares, female only care workers, due to cultural preferences.
Two days later, Jenny, Mrs Barnes' daughter asks you why her mother's arm is swollen and that she is not able to move it. Mrs Barnes also is sitting 'stiffly' and looks like she is in 'agony'. When informed about the fall, Jenny makes a formal complaint to the nurse manager and requests an urgent review. It is found that Mrs Barnes has a fracture in her arm, and she requires surgical intervention for repair of the fracture.
In arranging transfer to the hospital for health care, Jenny advises that Mrs Barnes does not have an advanced care directive. Jenny does not wish to 'decide' for her mother, and Mrs Barnes does not have capacity to decide currently. Jenny states that she is sure that her mum would 'be a donor' if there were any organs of any 'use to anyone'.
