Create a Health Promotion Plan on B.P. is a 39-year-old male who works as a Physician Assistant in my institution which is a nursing home. B.P. has been married for 15 years and has two children, a 7-year-old daughter and a 5-year-old son. He is of Caucasian descent, born in New York City and lives in a residential neighborhood near to his workplace and family. B.P. is a non-smoker and drinks alcohol sometimes during social or family events. B.P. volunteers at a local animal shelter twice a week, demonstrating his love for animals and dedication to community service. This should be appropriate, evidence-based interventions designed to address individual patient needs including, but not limited to: exercise, nutrition, dietary and activity counseling, weight management, smoking cessation, contraception, counseling on safe sexual practices, immunizations, chemoprophylaxis, stress management, and complementary and alternative therapies.
