Problem: DR is a 52 year old male taking phenytoin 600 mg/day (200 mg PO TID) as an anticonvulsant since 2012. You prescribed fluvoxamine 50 mg a week ago and increased his dose with another 50 mg yesterday in keeping with the practice of raising the dose every 4 to 7 days until a maximum therapeutic benefit is achieved. You receive a call that DR is in the emergency room after an episode of vomiting, dizziness, drowsiness and a skin rash. Did your prescribing fluvoxamine result in DR's hospitalization? How?
