Assignment Instructions

You wilt use the community and the health outcome/condition you identified in IvICD 3 (Morbidity and Mortality DISCUSSION) in conjunction with the "wheel" of the 10 Essential Public Health Services (from M1) to complete this assignment.

In a two page (max) document, you will:

1. Find and list S sources of high-quality literature that intersect 1) your community and 2) health outcome/condition and 3) your areas of personal and professional interest.

2. Define the health status of your community (include health care and social determinants). Using the 'wheel" Fixure from this CDC resource as a guider diagram all of the relevant public, private, and voluntary entities that contribute to the delivery of essential public health services within your selected community. You can diagram this using the computer ar a hand drawing. Once completed, on your diagram, include some type of visual representation to identify where you see your professional role in public health, In the diagram, include the following elements:
