Jane a 55-year-old Caucasian woman has been admitted to an acute psychiatric ward, she has a diagnosis of psychotic depression and recently stopped taking sertraline and olanzapine. On presentation, she has a tremor and akathisia and believes that government spies are after her. She is overweight and smokes about 15 cigarettes a day. She has a dry cough, and a temperature of 39°C and she is slightly breathless. There is a history of cardiovascular disease in Jane's family, and she worries because her father died from heart disease at the age of 53. It is believed that Jane's symptoms are consistent with suspected Covid infection, but a formal screen has to be conducted to confirm this. Jane was offered but declined to have the Covid vaccine three weeks ago, believing that the vaccine is poisoned. Jane's floating support worker reports to the ward that she has been over-eating and is always tired, lacking energy to do activities at home and outside.
