Question

What is the diagnosis of a patient with 48 year old female with a history of ADHD, depression, and bulimia presents today to establish care. She reports that she has run out of medciation since her last visit with her psychiatrist and he has subsequently passed. She feels that the medications she was on worked for her especially when she goes through rough patches. Patient states she is sad most of the day, have poor appetite and cannot sleep at nights. The symptoms are made worse when she gets stressed at work and it makes her want to isolate herself and quit. The patient reports the symptoms are relieved when she plays games on her phone. Patient states she feels better when she sees pictures of animals. The timing of the sadness is usually felt when she is by herself and her mind is not occupied. The patient states she is sad that her doctor passed but she knows that is apart of life. She denies any pain. She states she does not want her depression to get worse where she cannot sleep.
