Patient Report Some difficulty moving food back in the mouth recently Two-month history of "slurred" speech and difficulty with pronunciation Examination Results Normal neurological evaluation, except for palatal and tongue weakness Tongue moderately weak bilaterally-protrusion and lateralization were limited Possible fasciculations on the right side of the tongue Slight nasal emission present during pressure-sound production Mild hypernasality during conversational speech Imprecise consonant articulation, particularly for lingual consonants Vocal quality within normal limits (WNL) Sustained prolonged "ah" for 20 seconds Speech AMR for "puh" was normal, but "tuh" and "kuh" were slow No significant deterioration of speech with stress testing.
