Question

The next morning the gastroenterologist informs Mr. Gonzalez that their EGD confirmed a diagnosis of Barrett's esophagus with Dysplasia. The provider explains that it is a pre-cancerous stage in where the cell develops abnormal features. However, these abnormal cells do not have the capability to spread to other parts of the body. Biopsies were sent to determine the treatment. The client is being prepared for discharge and their IV catheter has been removed When the nurse returns to the room the client informs the nurse that when they went to the bathroom they became very lightheaded. Client also states their throat is still very sore. The nurse performs tilt test. Client vital signs: Lying flat: Temperature; 36.9 C (98,5 F): blood pressure: 118/62 mm Hg, heart rate: 92/min, respiratory rate 20/min, oxygen saturation 97% on room air. Sitting: Blood pressure: 109/60 mm Hg, heart rate 114/min Standing: Client becomes very lightheaded, and the nurse places them back into bed The provider advises the nurse to draw a stat CBC, administer a liter bolus of sodium chloride, and repeat CBC following administration of IV bolus. Client is to stay for another 24 hr for observation.
