He reports he is on adderall XR 30 mg twice a day, clonazepam 2 mg BID, Seroquel 300 mg at night and 100 mg in the morning.Review of crisp confirmed patient has been receiving these medication for a long time. he Reports excess worry, restlessness and feeling edgy, becomes easily fatigued, has muscle tension, decreased sleep, and decreased concentration when not on his medications.He further reports decreased focus and unable to complete tasks when not on his ADHD medications.
