Yasmin, a 32-year-old married woman, presents to your office. She was diagnosed with rapid-cycling bipolar disorder a year ago and was prescribed lithium 600mg/day. There was no improvement of symptoms over her six-month treatment period on lithium. Since that treatment was unsuccessful, she was referred to you. Yasmin's Office Visit Can you describe your symptoms to me? Sure. For the past two years, I've been so irritable and anxious. I can't sleep, and all I want is sweets. I have a really hard time focusing on getting things done. The funny thing is, it doesn't last forever. I feel like it's two weeks on, two weeks off, two weeks on, two weeks off. I feel bad about myself, and I'm so frustrated. As soon as I start to feel better, it starts back up again! How do these symptoms affect you socially? It really impacts my relationship with my husband. I take it all out on him, and I feel guilty. I just know he resents me for it and just wants me to be normal. It got worse when I was fired from work a year ago because I couldn't concentrate. That's when I was diagnosed with bipolar.
